No bonus registration is required. Limit one survey per dine. However, there's no limit to the number of times you can earn a Dining Survey Bonus or the amount you can earn. The survey only takes about a minute to complete and you have 30 days after your qualified dining transaction to complete it.

Restaurant Receipt

1Qualified dines for this bonus offer are dines at any participating restaurant, bar, or club that take place between 9/1/09 and 9/30/09 for which a Upromise contribution was earned. Offer is limited to members who are Preferred Diners at both the time of the qualified dine and upon completion of the survey. Limit one survey per dine. The survey must be completed within 30 days of the qualified dine in order to earn the $1 bonus. Please allow 4 - 6 weeks after you have completed your survey for any bonus rewards to post to your account. A minimum $15 purchase applies (including tax and tip) to earn the $1 bonus.
